Pre-Admission Information

Pre-admission is an important part of your procedure centre care. Your specialist will explain your procedure and complete the consent form with you during your consultation.

Pre-admission form

To ensure we can confirm your admission, health insurance and other details, please complete our online preadmission form at least 7 days before your admission.

If you would prefer not to complete the admission form online, you can email or fax us the paper forms usually provided to you by your surgeon. If you do not have any paper forms, please contact us prior to your admission date.

Pre-admission assessment

Please notify us if your weight is over 130kg prior to your admission as we will need to organise a pre-admission assessment.

Private Health Insurance

If you have private health insurance, please contact your health fund to check your level of cover for hospital admission.

We recommend you ask the following questions:

  • Am I fully covered for an admission to a private hospital?
  • Do I have an excess or any co-payments associated with my policy that I need to pay? 
  • Have I served all current waiting periods?
  • Are all my premium payments up to date?
  • Does my policy have any restrictions, limitations or minimum benefits that may apply to my planned procedure?
Reminder
  • If you have recently changed your health fund, or changed your level of cover with the same health fund, does this new change cover this admission and procedure?
  • Have you told your health fund if the procedure is related to an accident as it may be covered by another insurer such as WorkCover?

WorkCover or a Third-Party Claim

If your procedure is subject to WorkCover or a Third-Party Claim, please send full details of the claim to the procedure centre, including a letter from your insurance company accepting liability for your admission to hospital for the procedure.

One or two days prior to admission a staff member will contact you to confirm your admission, fasting requirements before the procedure, your general medical history, any medications you are currently taking and any hospital costs to be paid on admission.
