Police Checks

Applying for Criminal (Police) Record Check


In accordance with the new ART Act 2008, both partners are required to provide Melbourne IVF with an original Criminal Record check, in order to commence Assisted Reproductive Treatment (ART), after the 1st January 2010.  Criminal Record checks must be dated within 12 months of treatment commencement.

The application form can be downloaded from the Victoria Police Website:
www.police.vic.gov.au, and by following the steps below:
 

1.  Click on ‘Our services’
 

2.  Click on ‘Police Record Checks’
 

3.  Go to Downloads section (on right hand column) and click on Application form for National Police Record check April 2009’.  A pdf document will download.
 

Please note: to ensure that your Criminal Record check is admissible for ART treatment, please tick the box:
 

  • Section E
  • Option 1
  • Contact with Children
  • Please also write in the free text box under Option 1 ‘IVF Applicant’

 
 


Once you have received your Criminal Record check, we welcome you to either give this to a Melbourne IVF Patient Liaison Administrator OR alternatively, post it to:

The Criminal Record check will be returned to you once all administrative processes have been completed. 


Please note, if you have lived overseas in the last 10 years (for 12 consecutive months), further requirements are needed with regards to the Criminal Record check.  We encourage you to talk to a member of the Melbourne IVF Patient Liaison Administration team with regards to the details.  There is also information available on the government website: www.health.vic.gov.au
 

2 .  Child Protection Order Check Process

In accordance with the new ART Act 2008 and DHS policy effective from 1st January 2010, the process for patients wishing to apply for a Child Protection Order from January 2010 is outlined below:
 

1.  Download and print from Department of Human Services website:  “Request for child protection order check” application form


2.  Download and print from Melbourne IVF website:  “Consent to authorise Melbourne IVF to request Child Protection Order Check” form. 


3.  Alternatively, copies can be obtained from any Melbourne IVF clinic. 


4.  Return the Request for Child Protection Order Check application form AND Consent to authorise Melbourne IVF to request Child Protection Order Check form to Melbourne IVF in person or by posting to:

6.  Melbourne IVF will forward your Request for Child Protection Order Check application forms to the DHS.


7.  Once processed, the DHS will send the Child Protection Order Check statement directly to Melbourne IVF.

3.  Processing of Criminal Record Checks and Child Protection Order Checks


Once Melbourne IVF has received both your Criminal (Police) Record and Child Protection Order checks, we will process the checks in accordance with the ART Act 2008 legislation, and will contact you as soon as possible should we require any further information, prior to commencement of your treatment.
